NYC/NYS standard forms (e.g. the CH205) are printed and sent electronically through the patient portal at the day of a well child visit. Requests for additional or school/camp specific forms can be made on the portal or by calling the office. Requests for transfer of a child's complete medical record to other doctors need to be made in writing by one of the parents.
