If you are uncertain if our practice participates with your plan, please use our Tax ID (061506254) to inquire with your insurance carrier. Prior to your appointment, please call your insurance company to verify your specific plan's coverage of visits with our providers.


Please have your card with you at the time of your visit. For participating insurances, you are responsible for the co-pay and any other patient responsibilities as dictated by your insurance plan at the time of the visit. If we not are submitting to your insurance, you will be responsible for the entire charge at the time of service.
